Free Through Recovery Provider Guide - Region V

Each of the providers listed below offers Free Through Recovery services. This includes care coordination and recovery support services for individuals with serious behavioral health diagnoses and who are involved with the criminal justice system.

An individual being referred to Free through Recovery needs to meet the eligibility requirements of Free through Recovery ONLY, not the eligibility requirements of the provider’s already existing services.

Community Options

Community Options exists to empower people to express their dreams, and take responsibility to make and carry out their own decisions. We offer a person-centered approach and specialize in providing services to the following populations: Low Income, Single Parents, Dual Diagnosis, Brain Injury, Mental Health, Intellectual Disabilities, Developmental Disabilities and more. Community Options Services include Individual Planning and Support, Career Development and Financial Literacy.

Lutheran Social Services

Living out our mission to bring healing, help and hope, Lutheran Social Services of North Dakota is committed to helping people thrive at all stages of life by assisting people in finding their way – in times of trouble, in times of uncertainty, and in times of new beginning. LSSND has been doing this work in many service areas for 100 years and we are an accredited agency with offices in Fargo, Grand Forks, Bismarck, Minot, Devils Lake, Dickinson, Jamestown and Williston, and have highly qualified staff working in communities across the state. Our goal is to help make a successful transition to living in the community and will work with you on any barriers, such as housing, employment, and recovery to name a few. Our trained Care Coordinators in the Free Through Recovery program would like to assist you in meeting your goals, working through challenges and enjoying successes.

Redemption Road

Redemption Road Ministries is a Christian Faith based program that offers safe and sober housing for men and women in recovery from drug and/or alcohol addiction. We specialize in working with those individuals who are striving for successful, lasting and productive re-entry from jail or prison. Along with sober living, we focus on other vital areas of life in recovery such as employment, service opportunities in the community, and regular attendance and participation in a 12 Step recovery program like AA, NA or Celebrate Recovery. Our staff, with many combined years of successful and productive recovery, and also a long history of working with individuals coming out of our criminal justice system, will work hard to prepare our clients for success on both a personal and spiritual level as they recover from their addictions and re-enter society. STAND

Non-profit organization committed to the improved safety of North Dakota communities through the provision of high quality, evidence based sex offender treatment for individuals convicted or adjudicated of sexual crimes. We do not offer medication management or substance abuse treatment on site. Currently, all our FTR care coordinators are licensed mental health providers.

Lighthouse Church

Lighthouse Church of Fargo, ND is a faith community where no perfect people are allowed! We believe everyone makes bad decisions at times which can lead to difficulties in life, but God provides the foundation to solve life’s obstacles. We are confident in His abilities to do for you what he has done for us. We offer many programs designed to equip individuals with spiritual solutions to life’s difficulties. In addition to regular church programming (worship services, Sunday School, youth groups, Bible studies, adult learning and fellowship opportunities), we offer recovery programming(Celebrate Recovery, Spirituality Recovery group, Depressions groups). The Lighthouse is a host site for a variety of AA, NA, Al Anon, and Nar Anon meetings. The Lighthouse has a family feel and serves as a life community for many who are seeking recovery while putting the pieces of their lives back together. We understand the difficulty involved in changing your life in so many ways in order to move forward one day at a time. The Lighthouse can’t do it for you, but we can be a place of love, encouragement, hope and recovery as you work to improve your life!

F5 Project

F5 Project provides transitional and re-entry support for those who were, and are incarcerated before they are released. We pride ourselves on working on the inside to help bridge to the community. We help prepare people with a specialized entrepreneur mindset taught by those who were formerly incarcerated. We believe this help with character development, parenting, employment readiness, and social engagement.

We believe that not only is the person being transformed, but our community as well. That is why we have community meetings where our staff, the people coming out of incarceration and the community can engage one another and destroy stigmas. These connections heal worldviews, families, and communities.

As part of the transition and re-entry support, we provide safe and affordable housing with peer mentorship. The peer will help with employment support, positive social networks, and transportation. A peer is defined as someone who was formerly incarcerated that has lived in the shoes of the person they are providing support for.

Fraser LTD

The mission of Fraser Ltd is to support individuals on their life’s journey to independence. Our core services provide the basic care needed for a healthy life. This includes safe, affordable housing; teaching independent living skill; crisis management - including Valley Hope counseling center open to both the public and internal clientele in crisis; representative payee services (financial literacy/management) for adults ages 18 and older; and at-risk youth ages 16 to 26 years of age. We are a housing-first model which focuses on person-centered care, and we support individuals six weeks to end of life. We serve a broad range of individuals across our three major programs including children typically developing and those with special needs, as well as those on the autism spectrum. Youth at risk of homelessness, incarceration, trafficking, and adults with intellectual, developmental disabilities, mental health and behavioral issues are served in our residential and day programs. The Stepping Stones Resource Center (SSRC) is a part of the Fraser Continuum of Care model, providing drop in and emergency care for the youth in the community.

Freedom Through Recovery

Need help finding housing, a job/career, training and/or education, substance use treatment (inpatient/outpatient), mental health services, obtaining your driver’s license, community health services, family reunification, support groups (Alcoholics Anonymous, Gambler’s Anonymous, Narcotics Anonymous), recovery worship, grief counseling or financial management services?

For the person seeking to change their life in a positive manner, our years of experience have helped direct individuals to the proper resources. By utilizing a person-centered approach, the Freedom Through Recovery team will provide advocacy, peer support, resource navigation and ongoing case management.

Elisha Hoye, B.S. has 10+ years of first-hand experience working with juveniles and adults. Her expertise is helping individual clients earn their high school diploma/GED, seeking employment/training opportunities and locating affordable housing. Her mission focuses on implementing positive behavioral platforms for each to achieve their short term and long-term goals.

Elliott Kabanuk, B.A, M.A. stresses the importance of one-on-one interaction and personalized assessment, career planning, and short and long-term goals. The foundation of his philosophy is person-centered and rooted in his 30+ years of recovery. Elliott is a licensed Alcohol and Drug Counselor (in Minnesota), ICGC-1 (International Certified Gambling Counselor) and an employment specialist.

Face it TOGETHER

We help addiction sufferers and their loved ones get well and stay well through peer addiction management coaching and other support. When clients step through our doors, we want them to know that they’re in the right place. We help take the mystery out of this disease. Our coaches are highly trained peers who have walked in your shoes. They provide practical, goal-oriented support and guide addiction sufferers and their loved ones through the wellness process. We’re independent and non-judgmental and will support any path to getting well. We also connect families to resources and services in the community, including treatment, housing, employment support and other needs to help them stay on track. Our coaching is confidential and available to anyone for any length of time. We serve clients at any stage of recovery. Our coaches will help those impacted by drug and alcohol addiction find their way to wellness.

Community Medical Services

Community Medical Services (CMS) provides medication assisted treatment as an Opioid Treatment Program in Fargo and Minot, North Dakota. CMS is a level 1 outpatient substance abuse treatment program, which includes counseling, case management, and a medical provider, with options for medications including methadone, buprenorphine (suboxone), and Vivitrol. Intakes can be scheduled or walk-ins are welcome. Normal clinic hours are 6am to 11am Monday through Friday, with dosing provided on Saturdays from 7am to 9am.

Lost and Found Recovery Center

Lost and Found Recovery is a program of Trinity Lutheran Church in Moorhead, MN. They offer individual and family counseling, Recovery services and support, family support groups and Chemical health and mental health evaluations. Services and Support provided to individuals is personalized. They are staffed by professional counselors licensed in addiction counseling and mental health.

Solutions Behavioral Healthcare Professionals

Solutions Behavioral Healthcare Professionals has a full range of mental health professionals, including psychiatrists, psychologists and licensed therapists. An array of services are available, including individual, family, group, and marital therapy, as well as consultation. A collaborative treatment plan is developed to address the areas of concern, based upon an assessment of the problem area. Our therapists are trained in evidence-based practices, which means that they provide those types of programs, interventions, and therapies which have been demonstrated to be effective. Examples of evidence-based therapies provided at Solutions include cognitive and behavioral therapy (CBT), Parent-Child Interaction Therapy (PCIT), Dialectical Behavior Therapy (DBT), and Trauma-Focused Cognitive Behavior Therapy (TFCBT), Prolonged Exposure Therapy (PTSD related treatment for adults), Cognitive Processing Therapy (PTSD related treatment for adults). Incredible Years programming is provided for children and families. Individual treatment plans incorporate meaningful and practical goals.

Solutions Fargo office provides intensive services through ND Rehabilitative Services for those who qualify under ND Medicaid. These services are intensive and comprehensive and are designed to prevent hospitalization, incarceration, severe family disruption and relapse in general. They are intended to be short term and intensive and to reduce the immediate risk and can be followed up with less intensive services as necessary. This programming serves the entire family if necessary and services can be provided in the office, home, school, daycare, etc. to meet the needs of the individual and family. Our Fargo office is near a public transportation bus route which makes it more accessible to those without other means of transportation. We are also within walking distance of Parole and Probation and Cass County Jail and are available to provide services at either of these locations if it is requested.

Community Uplift Program

The Community Uplift Program (CUP) is a nonprofit agency that works to bolster the visibility of minority populations in our community. The primary focus is the LGBTQ+ population and people of color, we work holistically with area organizations to help anyone who asks. Within CUP we also house the Harbor Health Clinic, which is sliding fee scale clinic that offers primary care services to the community and specializes in hormone therapy for transgender individuals.
